The Author

Dr. Michael Crawford, chief of clinical cardiology at UCSF Medical Center, specializes in treating coronary artery disease, valvular heart disease and heart failure as well as congenital heart disease in adults and heart disease in athletes.

Crawford earned a medical degree at the University of California, San Francisco. He trained in internal medicine at UCSF Medical Center and Beth Israel Hospital in Boston, followed by cardiology training at the University of California, San Diego, where he joined the faculty. He was co-director of cardiology at the University of Texas in San Antonio and chief of cardiology at the University of New Mexico in Albuquerque. Before returning to UCSF, Crawford was a consultant and vice chair of cardiology at the Mayo Clinic in Scottsdale, Ariz. Crawford has served as chair of the Council on Clinical Cardiology of the American Heart Association and as a board member of the American College of Cardiology. He is a fellow of the American College of Physicians and a member of the Association of University Cardiologists and Western Association of Physicians.

  1. Well written. Reasonably detailed without being overwhelming. Some chapters like cardiac amyloidosis, adult congenital heart disease (for example) were too superficial for a cardiology fellow, but are probably adequate for internal medicine. Good as a review book and also if you want to look up stuff quickly. It’s one step above a manual in that the explanations are quite detailed.

  2. Difficult subject matter, but this book breaks critical care topics down making the subjects enjoyable to read! Helpful for new PAs entering the ICU.

  3. I am liking this book. Its a bit broad in subject matter obviously but still good reading. Its a nice reference to have at home. Usually I find myself looking stuff up after I get home from taking care of an especially sick patient. If you work in critical care I would reccommend this book.

The Author

Dr. Doherty, an acclaimed endocrine surgeon, is a graduate of Holy Cross and the Yale School of Medicine. He completed residency training at UCSF, including Medical Staff Fellowship at the National Cancer Institute.  Dr. Doherty joined Washington University School of Medicine in 1993, and became Professor of Surgery in 2001. In 2002 he became Head of General Surgery and the Norman W. Thompson Professor of Surgery at the University of Michigan, where he also served as the General Surgery Program Director and Vice Chair of the Department of Surgery. From 2012 to 2016, Dr. Doherty was the Utley Professor and Chair of Surgery at Boston University and Surgeon-in-Chief at Boston Medical Center before becoming Moseley Professor of Surgery at Harvard Medical School, and Surgeon-in-Chief at Brigham and Women’s Hospital and Dana-Farber Cancer Institute.

Dr. Doherty was trained in Surgical Oncology, and has practiced the breadth of that specialty, including as founder and co-director of the Breast Health Center at Barnes-Jewish Hospital.  His clinical and administrative work was integral in the establishment of the Siteman Cancer Center at Washington University.  Since joining the University of Michigan in 2002, he has focused mainly on surgical diseases of the thyroid, parathyroid, endocrine pancreas and adrenal glands as well as the surgical management of Multiple Endocrine Neoplasia syndromes. He has devoted substantial effort to medical student and resident education policy.  His bibliography includes over 300 peer-reviewed articles, reviews and book chapters, and several edited books.

Dr. Doherty is Past-President of both the International Association of Endocrine Surgeons (IAES) and the American Association of Endocrine Surgeons (AAES). He currently serves as Treasurer of the International Society of Surgery (ISS), the Historian for the Society for Clinical Surgery (SCS) and Editor-in-Chief of AccessSurgery.

The Author

Darren K. McGuire, M.D., M.H.Sc., is a Professor in the Department of Internal Medicine at UT Southwestern. He is also the Director of the Parkland Hospital and Health System Outpatient Cardiology clinics. Dr. McGuire earned his medical degree at the Johns Hopkins University School of Medicine and completed an internal medicine residency at UT Southwestern. He then received advanced training in cardiology through a fellowship at Duke University, where he also conducted research for two years, working on a number of large-scale, international clinical trials and registries at the Duke Clinical Research Institute (DCRI). He also earned a master’s degree in health sciences in clinical trials at the Duke University School of Medicine. He joined the cardiology faculty at UT Southwestern in 2001. His main clinical and research interests are in the long-term prevention of and risk-modification for cardiovascular disease, especially among patients with diabetes. Dr. McGuire is Deputy Editor of Circulation, Senior Editor of Diabetes and Vascular Disease Research, and editor of the textbook Diabetes in Cardiovascular Disease: a companion to Braunwald’s Heart Disease. He is a Fellow of the American Heart Association (AHA), the American College of Cardiology and the European Society of Cardiology; a former Chair of the AHA Diabetes Committee; and a former member of and present ad hoc consultant for the FDA Cardiovascular and Renal Drugs Advisory Committee. Dr. McGuire has received numerous teaching awards, most notably the UT System Board of Regents Outstanding Teaching Award in 2013.

Lisa A. UrryLisa Urry (Chapter 1 and Units 1, 2, and 3) is Professor of Biology and Chair of the Biology Department at Mills College in Oakland, California, and a Visiting Scholar at the University of California, Berkeley. After graduating from Tufts University with a double major in biology and French, Lisa completed her Ph.D. in molecular and developmental biology at Massachusetts Institute of Technology (MIT) in the MIT/Woods Hole Oceanographic Institution Joint Program. She has published a number of research papers, most of them focused on gene expression during embryonic and larval development in sea urchins. Lisa has taught a variety of courses, from introductory biology to developmental biology and senior seminar. As a part of her mission to increase understanding of evolution, Lisa also teaches a nonmajors course called Evolution for Future Presidents and is on the Teacher Advisory Board for the Understanding Evolution website developed by the University of California Museum of Paleontology. Lisa is also deeply committed to promoting opportunities for women and underrepresented minorities in science.

Michael L. Cain

Michael Cain (Units 4, 5, and 8) is an ecologist and evolutionary biologist who is now writing full-time. Michael earned a joint degree in biology and math at Bowdoin College, an M.Sc. from Brown University, and a Ph.D. in ecology and evolutionary biology from Cornell University. As a faculty member at NEW! Mexico State University and Rose-Hulman Institute of Technology, he taught a wide range of courses, including introductory biology, ecology, evolution, botany, and conservation biology. Steve Wasserman (Unit 7) is Professor of Biology at the University of California, San Diego (UCSD). He earned his A.B. in biology from Harvard University and his Ph.D. in biological sciences from MIT. Through his research on regulatory pathway mechanisms in the fruit fly Drosophila, Steve has contributed to the fields of developmental biology, reproduction, and immunity. As a faculty member at the University of Texas Southwestern Medical Center and UCSD, he has taught genetics, development, and physiology to undergraduate, graduate, and medical students. He currently focuses on teaching introductory biology. He has also served as the research mentor for more than a dozen doctoral students and more than 50 aspiring scientists at the undergraduate and high school levels. Steve has been the recipient of distinguished scholar awards from both the Markey Charitable Trust and the David and Lucille Packard Foundation. In 2007, he received UCSD’s Distinguished Teaching Award for undergraduate teaching.

Peter V. Minorsky

Peter Minorsky (Unit 6) is Professor of Biology at Mercy College in New York, where he teaches introductory biology, evolution, ecology, and botany. He received his A.B. in biology from Vassar College and his Ph.D. in plant physiology from Cornell University. He is also the science writer for the journal Plant Physiology. After a postdoctoral fellowship at the University of Wisconsin at Madison, Peter taught at Kenyon College, Union College, Western Connecticut State University, and Vassar College. His research interests concern how plants sense environmental change. Peter received the 2008 Award for Teaching Excellence at Mercy College.

Jane B. Reece

The head of the author team for recent editions of CAMPBELL BIOLOGY, Jane Reece was Neil Campbell’s longtime collaborator.  Earlier, Jane taught biology at Middlesex County College and Queensborough Community College. She holds an A.B. in biology from Harvard University, an M.S. in microbiology from Rutgers University, and a Ph.D. in bacteriology from the University of California, Berkeley. Jane’s research as a doctoral student and postdoctoral fellow focused on genetic recombination in bacteria. Besides her work on the Campbell textbooks for biology majors, she has been an author of Campbell Biology: Concepts & ConnectionsCampbell Essential Biology, and The World of the Cell.

Neil A. Campbell

Neil Campbell (1946–2004) combined the investigative nature of a research scientist with the soul of an experienced and caring teacher. He earned his M.A. in zoology from the University of California, Los Angeles, and his Ph.D. in plant biology from the University of California, Riverside, where he received the Distinguished Alumnus Award in 2001. Neil published numerous research articles on desert and coastal plants and how the sensitive plant (Mimosa) and other legumes move their leaves. His 30 years of teaching in diverse environments included introductory biology courses at Cornell University, Pomona College, and San Bernardino Valley College, where he received the college’s first Outstanding Professor Award in 1986. He was a visiting scholar in the Department of Botany and Plant Sciences at the University of California, Riverside. Neil was the lead author of Campbell Biology: Concepts & ConnectionsCampbell Essential Biology, and CAMPBELL BIOLOGY.

Viruses are studied either because they cause significant human, animal or plant disease or because they are useful materials for probing basic phenomena in biology, chemistry, genetics and/or molecular biology. Arenaviruses are unusually interesting in that they occupy both categories. Arenaviruses cause several human diseases known primarily as the hemorrhagic fevers occurring in South and Latin America (Bolivia: Machupo, Argentine, Junin virus, and Brazil: Sabia virus) and in Africa (Lassa fever virus). Because such viruses produce profound disabilities and often kill the persons they infect, they are a source of health concern and economic hardship in the countries where they are prevalent. Further, they provide new problems for healthcare persons owing to the narrowing of the world as visitors from many countries travel increasingly to and from endemic areas and may incubate the infectious agent taking it from an endemic area into an area where the virus is not expected. Such cases are now being recorded with increasing frequency. In addition to these hemorrhagic fever viruses, the arenavirus lymphocytic choriomeningitis virus (LCMV) can infect humans worldwide,although the illness is most often less disabling and severe than those elicited by the other arenaviruses. Yet, LCMV is of greater concern to non-arenavirologists and experimentalists using tissue culture or animals, etc., because normal-appearing cultured cells or tissues from animals used for research may be persistently infected with LCMV without manifesting clinical disease or cytopathology and may transmit that infection to laboratory workers·. For example, in 1975 Heinemann et aI., recorded 48 cases of LCMV infection among personnel in the Radiotherapy Department and vivarium at the University of Rochester School of Medicine. These persons had contact with Syrian hamsters into which tumors, unknown to be infected with LCMV, were injected. The tumor cells were obtained from an outside, well-known, research supplier who distributed tumor cell lines to numerous laboratories primarily interested in SV40 and polyoma virus research. A subsequent investigation of the 22 tumor lines revealed that 19 yielded in fectious LCMV. Thus, both production of SV 40 and/or polyoma virus from these cells or the use of hamsters infected with such tumors provided an unanticipated human biohazard. In the Rochester scenario infection was spread from those doing basic research, presumably via air ducts to multiple individuals at the medical school/hospital including healthcare workers and patients undergoing radiologic procedures. Similarly, the Centers for Disease Control (CDC), USA, has recorded multiple cases of LCMV infection in families scattered from the northeast coast of the United States (New York) to the western region (Reno, Nevada) originating from hamsters sold by a single pet store supplier in the southeast (Florida). In addition, several investigators found that their hybridomas making monoclonal antibodies were infected with LCMV. The likely source was infected spleen feeder layers from clinically healthy but persistently infected mice purchased from a well-known commercial source which supplies mice to many universities and institutes in North America. Thus, researchers studying such diverse areas as the molecular biology of SV 40, biological effects of Chlamydia, immune response of mice, and producing hybridoma cells or ascites fluids have found their preparations contaminated with LCMV. In these cases, the potentially dangerous effects of LCMV were not anticipated because the virus does not cause footprints associated with most acute viruses (i.e., n~crosis, inflammatory response). The recognized arenaviruses of man, vectors and laboratory models used for their study are listed in Table 1.

The Atlas of Immunology is designed to provide a pictorial reference and serve as a primary resource for the most up-to-date and thorough illustrated treatise available in the complex science of immunology. The book contains more than 1,000 illustrations and depicts essentially every concept of importance in understanding the subject matter of immunology. It is addressed to immunologists and nonimmunologists alike, including students, researchers, practitioners, and basic biomedical scientists. Use of the book does not require prior expertise. Some of the diagrams illustrate basic concepts, whereas others are designed for the specialist interested in a more detailed treatment of the subject matter of immunology. The group of illustrations is relatively complete and eliminates the need to refer to another source. The subject matter ranges from photographs of historical figures to molecular structures of recently characterized cytokines, the major histocompatibility complex molecules, immunoglobulins, and molecules of related interest to immunologists.

In any movement of their life, immune cells, especially T and B lymphocytes, are confronted with an essential choice: to continue their existence or to commit a sort of metabolic suicide that is referred to as apoptosis or programmed cell death. In contrast to most philosophers, lymphocytes and their precursors are constantly susceptible to suicide, and it even appears that the usual cause of T or B cell elimination is suicide rather than death from natural causes, accidents or murder. This book provides a vast overview of lymphocytes suicide: external triggers and internal motives leading to suicidal impulses, accomplices in self-destruction, weapons implicated in self-execution, removal of dead bodies and pharmacological prevention of suicide.
Most of the chapters in this book are devoted to the physiology of apoptosis. The goal is to unmask the external triggers of apoptosis, unravel the signal transduction processes involved therein and describe the role of oncogenes, “death genes” and effector molecules in the apoptotic cascade. The remaining chapters deal with the pathophysiological aspects of lymphocyte apoptosis, namely, as a host contribution to HIV-induced lymphopenia, and therapeutic strategies for the avoidance of lymphocyte death.
We are confident that this compendium will contribute to the exploration of cellular suicide, not only from a basic scientist’s viewpoint but also with regard to the possible clinical implications of apoptosis (dys)regulation. Far from having a depressing effect on the reader, cellular suicide may thus provide a source of both intellectual excitement and therapeutic inspiration.

Antigen presentation is a subject most often considered at the molecular level, but its results are, of course, also relevant to the whole organism. This compilation of chapters from experts is aimed at reviewing the broad spectrum from molecular, biochemical and pharmacological considerations through to the effects on host immunity and the diseases which result from defects in these pathways. We begin with the class I MHC pathway. Drs. Nandi, Marusina and Monaco review how the endogenous peptides are generated within the cell and how they are transported into the endoplasmic reticulum. There, the story is taken up by Drs. Fourie and Yang, who relate how the complexes are assembled and transported to the cell membrane. Drs. Hudrisier and Gairin then describe the structure ofthe mature class I complex and reflect upon the pharmacology and possible therapeutic use of peptide antagonists. The following two chapters illustrate the consequences of low class I MHC expression. The defects in immune responsiveness which result from globally defective class I presentation are reviewed by Drs. Frelinger and Serody, who describe the resultant defects in the ability to control challenge by different microbial pathogens, while Dr. RaIl argues that the restricted class I expression, seen in certain tissues of normal animals, is most probably beneficial to the host, preventing immunopathologically mediated cell death. Then, the interface between the host and several viral pathogens is addressed in Chaps. 7 and 8; Drs. Sparer and Gooding describe how adenoviruses interfere with antigen presentation and, thus, with the host’s ability to respond appropriately to the virus challenge. Drs. Johnson and Hill show how, even in a single virus family (the herpes viruses), many different approaches to immune evasion have been taken. In Chap. 9, Drs. Nordeng, Gorvel and Bakke provide an excellent review of the MHC class II pathway, demonstrating that the invariant chain has many and varied functions. In the final chapter, Drs. Eibl and Wolff discuss the consequences of a defective MHC class II pathway in transgenic mouse models and in humans.

Preface

This Book is published as a mid-term step (i.e., middle of every 3-year period) by the Allergy and Immunotoxicology Scientific Committee (AISC) of the International Congress of Occupational Health (ICOH). The comprehensive meeting of ICOH is held every 3 years. Recently, it was held in Seoul, South Korea, 31 May–5 June 2015, when Prof. Mario Di Gioacchino, of Chieti, Italy, served as the chairperson. The next meeting will be held in Dublin, Ireland, in 2018. In conjunction with the opening of the special, general, oral, and poster sessions as well as joint sessions with other scientific committees (SCs), several SCs are staging mid-term activities independently. Our SC/AISC has also been holding several special midterm symposiums in Italy, China, and Japan (in Kumamoto and Kyoto) and calls each such symposium an International Symposium on Occupational and Environmental Allergy and Immune Diseases (ISOEAID). As the current chairperson of this SC, Prof. Otsuki was the local organizer of ISOEAID 2010 held at Kyoto. It was April and we were surrounded by beautiful cherry blossom trees in full bloom. During the symposium, we presented and discussed many issues regarding the occupational and environmental allergy and immunotoxicology field and enjoyed the lectures on air pollution such as the effects of PM2.5 on immune organs, immunological aspects of sick-building syndrome, sensitizers in GHS (The Globally Harmonized System of Classifi cation and Labelling of Chemicals), and childhood and immunology.
The first honorary chairperson in our SCs was Prof. Toshio Matsushita, Kagoshima, Japan. Thereafter, Prof. Poalo Boscolo, Chieti, Italy, and Prof. Kanehisa Morimoto, Osaka, Japan, inherited the chairperson’s role. All three editors of this eBook have been in that role in our SC. Prof. Di Gioacchino served as Chair 2009–2014. I have been the Chair since 2015, with Prof. Petrarca as the secretary in the SCs. Subsequently we decided to publish this eBook instead of ISOEAID at this mid-term point based on the presentation in the ICOH meeting in Seoul, South Korea.
The AISC has been focusing on investigation and discussion of the pathophysiological mechanisms involved with the allergic and immunotoxicological effects of environmental as well as occupational substances. Particularly, occupational allergies such as dermatitis and asthma were practically classical occupational diseases. The basic strategy is to avoid exposure, whereas there have been numerous newly developed substances that are being introduced in occupational situations. Thus, establishing methods of prediction using in vitro and in silico procedures is required. In addition to these investigations, epidemiological studies are also important, as is establishing a system to quickly identify allergic diseases caused by newer substances, and it is valuable, as well, to prevent workers from contracting these diseases. In addition, with allergic diseases the immunotoxicological effects of various substances in the environment as well as occupational settings are also important. Major members of our SC have been investigating the immunotoxicological effects of fibrous and particulate matters such as silica, asbestos, and recently nanomaterials including nanoparticles, nanotubes, and nanosheets.
