Alexis Arzimanoglou (Author)

Alexis Arzimanoglou is the Director of the Paediatric Epilepsy and Functional Neurology Department at the University Hospitals of Lyon, France and Visiting Professor at the Universitat de Barcelona, Spain, coordinating the Epilepsy Research Program at the Hospital San Juan de Dios. He trained in Neurology at Great Ormond Street, London, UK and at the Hôpital de la SALPETRIERE and in Child Neurology as a fellow of Jean AICARDI at the Hôpital des Enfants Malades in Paris, France. He then with worked with Jean Aicardi for over 25 years. He served as Chair of the Scientific Committee of the European Paediatric Neurology Society, elected member of the European Commission of the International League Against Epilepsy (ILAE) and President of the Société Européenne de Neurologie. He is currently as International League Against Epilepsy Ambassador for Epilepsy for the ILAE, Editor-in-Chief of  the ILAE Educational Journal, Epileptic Disorders and  Associate Editor of the European Journal of Paediatric Neurology, official journal of the European Paediatric Neurology Society. He is the author of two books, editor of seven and an author or co-author of over one hundred scientific articles.

Gray’s Anatomy has been a cornerstone of medical education since its original appearance in 1858. It has provided a remarkably authoritative description of both gross and microscopic anatomy of the human body for many generations of medical students and practicing medical scientists on a worldwide basis. It has been, and remains, cherished not only as a primary source of anatomical knowledge but also as a reliable resource to which the student or practitioner might return for many years, indeed, throughout the entire length of a medical career. Although the classical text is regularly updated, recent major developments in both basic and clinical medicine have prompted a major reconsideration of the utility of a single large volume devoted to all of human anatomy. Concerns are especially related to the increasing specialization, if not frank fragmentation, of the medical arts with which the contemporary physician must deal on a day-to-day basis. As a consequence of such a reappraisal, a decision has been made to extract focused portions of the major text devoted to specific conceptual domains. Gray’s Anatomy itself will remain as authoritative as ever but will be expanded by the inclusion of clinical case material to illustrate in depth, whenever possible, the application of anatomical principles to the bedside. The field of neuroanatomy lends itself particularly well to such a departure from the more traditional approach to human anatomy, with the original Gray’s material being utilized as the foundation for such an enhanced pedagogical approach. In Gray’s Clinical Neuroanatomy, virtually all the original neuroanatomical text in the thirty-ninth edition is preserved, although it is transposed and rearranged to meet innovative structural guidelines and is complemented by a host of clinical case vignettes, which in turn are augmented by visual materials designed to strengthen the link between the clinic and the dissecting room. It must be emphasized that there has been no attempt to develop yet another comprehensive textbook of neurology as such; the neurological disorders cited here are entirely exemplary and directly relevant to the underlying anatomical principles of the traditional Gray’s.

I learn with dismay that in some medical colleges in Mumbai and elsewhere, education at the bedside is being given short shrift. Leisurely ward rounds conducted daily by senior physicians and surgeons along with a retinue of residents and students; several minutes being spent at each bedside for elicitation of details of history, demonstration of physical signs and discussion on diagnosis and care are now matters of the past. Likewise, discussions in the outpatient department, where rapid clinical assessment and triaging are best learnt by students and residents, are also disappearing. Finally, the bedside clinic, where the head of the clinical unit sat by the bedside of a selected patient with students and residents on stools around the bed, is also a mere memory. Medical students, present and future, are being immensely deprived due to the present practice of reliance on tests to the exclusion of the use of one’s clinical judgement. Works such as Applied Medicine—Descriptive Cases and Cases Demonstrated at the Bedside by Question and Answer (G.E. Beaumont, J. & A. Churchill, London, 1950) and Bedside Clinics in Medicine (F.E. Udwadia, Oxford University Press, Delhi, 2007) are now seldom encountered. Under these circumstances, this volume is warmly welcome. The headings of the chapters indicate the approach followed by editors and authors. The emphasis is on the person who is now a patient: a child with a large head, a child with an abnormally shaped head, a child with a swelling on the forehead and widened distance between the orbits, a patient with dementia, a patient with seizure, a patient with sudden severe headache, a patient with spastic paraparesis…
