The tenth edition is an important milestone in the life of a textbook. This occasion is a propitious time to look back on the origins of Basic Pathology, which are summed up best by quoting Stanley Robbins from the preface of the first edition (1971): “Of books as well as men, it may be observed that fat ones contain thin ones struggling to get out. In a sense, this book bears such a relationship to its more substantial progenitor, Robbins Pathology. It arose from an appreciation of the modern medical student’s dilemma. As the curriculum has become restructured to place greater emphasis on clinical experience, time for reading is correspondingly curtailed.…In writing this book, rare and esoteric lesions are omitted without apology, and infrequent or trivial ones described only briefly. We felt it important, however, to consider rather fully the major disease entities.” While the goals of “baby Robbins” remain true to the vision of Stanley Robbins, this edition has been revised on the basis of a few additional principles. • First, it is obvious that an understanding of disease mechanisms is based more than ever on a strong foundation of basic science. In keeping with this, we have always woven the relevant basic cell and molecular biology into the sections on pathophysiology in various chapters. In this edition we go one step further and introduce a new chapter titled “The Cell as a Unit of Health and Disease” at the very beginning of the book. In this chapter we have attempted to encapsulate aspects of cell and molecular biology that we believe are helpful in preparing readers for discussions of specific diseases. To this effect, the organization of this book follows that of most primary texts in the field and parallels the syllabus used in pathophysiology courses in medical schools throughout the United States. Ample space is provided for students to make notes during course study and while viewing the online videos that cover each section of the text (wwwpathoma.com). We recommend that students use Fundamentals of Pathology during their medical courses, taking notes in the margin as pertinent topics are covered. When exam time comes around, these notes will likely be invaluable. For examination preparation, we suggest students read the material first, then listen to the online lecture, and then reread the material to develop a solid grasp of each topic. One should not become disheartened if they are not able to retain all the information contained herein. This deceptively slim volume covers a tremendous amount of material, and repetition will be a key aid as you progress in your studies. An effort has been made to emphasize concepts and principles over random facts, the forest rather than the trees. Attention to the same by the student will provide a deeper, more meaningful understanding of human disease. We must always remind ourselves that ultimately our goal is to learn, to share, and to serve. Fundamentals of Pathology was developed with this goal in mind.

In this book we have set out to provide a practical guide to bone marrow diagnosis, based on an integrated assessment of peripheral blood and bone marrow aspirate films, trephine biopsy sections and supplementary investigations. We believe that a trephine biopsy specimen should not be examined and interpreted in isolation. We have therefore discussed the clinical context of bone marrow diagnosis and have given equal weight to cytological and histological features. Since bone marrow diagnosis is no longer based on morphological features alone, we have also discussed in detail the role of immunophenotypic, cytogenetic and molecular genetic analysis. We have incorporated the diagnostic criteria and terminology of the 2016 World Health Organization classification of tumours of haematopoietic and lymphoid tissues. We have dealt very fully with haematological disorders for which bone marrow examination is commonly performed. However, we have also sought to be comprehensive, including information on uncommon and rare disorders so that the book will serve as a useful reference source. When possible, we have illustrated rare as well as common conditions and have cited the relevant scientific literature extensively. As in the previous edition, magnifications are given as the microscopic objective used rather than the magnification on the printed page. Unless otherwise specified, trephine biopsy sections were embedded in paraffin wax. We have retained and expanded problems and pitfalls, diagnostic algorithms and summary boxes. We hope that haematologists, histopathologists and haematopathologists will continue to find Bone Marrow Pathology a useful aid in their day‐to‐day practice and that trainees in these disciplines will find it indispensable. Cytogeneticists, molecular geneticists and scientists working in flow cytometry laboratories should find that it provides them with a context in which to interpret their findings.

The source materials are the ninth editions of Robbins and Cotran Pathologic Basis of Disease (PBD9) and Robbins Basic Pathology (BP9), and in several chapters, Robbins and Cotran Atlas of Pathology (AP3). The questions in this review book follow the chapters and topics in these source materials to facilitate ongoing selfassessment as students work their way through a curriculum to gain and then apply their understanding of key concepts. This book is intended to be a useful resource for students in a variety of health science training programs. In keeping with recommended question writing style for licensing examinations, we have included single best-answer questions, most with a clinical vignette, followed by a series of homogenous choices. This approach emphasizes an understanding of pathophysiologic mechanisms and manifestations of disease in a clinical context. We have incorporated relevant laboratory, radiologic, and physical diagnostic findings in the questions to emphasize clinicopathologic correlations. Although this adds to the extent of individual questions, the thoroughness reinforces learning, as a review should. As we launch the ninth edition of Pathologic Basis of Disease we look to the future of pathology as a discipline and how this textbook can remain most useful to readers in the twenty-first century. It is obvious that an understanding of disease mechanisms is based more than ever on a strong foundation of basic science. We have always woven the relevant basic cell and molecular biology into the sections on pathophysiology in various chapters. In this edition we go one step further and introduce a new chapter at the very beginning of the book titled “The Cell as a Unit of Health and Disease.” In this chapter we have attempted to encapsulate aspects of cell and molecular biology that we believe are helpful in preparing readers for the more detailed discussions of specific diseases. We would like to remind readers that the last time a new chapter was added to this book was in 1967 when Stanley Robbins, at that time the sole author, decided to add a chapter on genetic diseases, one of many farsighted decisions by Dr. Robbins. We hope that the new chapter will be in keeping with his legacy. In the preface of the very first edition (1957), Stanley Robbins wrote: “The pathologist is interested not only in the recognition of structural alterations, but also in their significance, i.e., the effects of these changes on cellular and tissue function and ultimately the effect of these changes on the patient. It is not a discipline isolated from the living patient, but rather a basic approach to a better understanding of disease and therefore a foundation of sound clinical medicine.”

With Osborn’s Brain, irst edition, not only did I break a promise to myself that I would “never, EVER write another prose-based textbook,” I would have been stunned to know I’d actually write a second edition of the book! But here we are, ive years later, and I’ve done it. There have been so many changes in neuroradiology since the publication of the irst edition that I wanted—and needed—to do a thorough update. This edition is actually more than just an update; it is a major rewrite. Most of the 4,000+ images have been replaced with new, even better examples, and there are numerous new diagnoses that are presented. The Cancer Genome Atlas project has revolutionized our understanding of CNS neoplasms, and the exploding knowledge was relected in the latest World Health Organization’s Classiication of Tumours of the Central Nervous System, published just a year ago. The “4-plus” WHO forms the basis of the completely rewritten neoplasms section of this edition. For those of you who are new to Osborn’s Brain, both the irst and second editions are written as a curriculum in neuroradiology. The “must know—now!” topics such as brain trauma, stroke, and brain bleeds are covered in the irst few chapters of the text. The book is meant to be read cover to cover, progressing through all the major areas of neuroimaging and neuropathology. While the basics are emphasized, there is a great deal of “advanced” information (read: fellow-and-beyond level stuf) that practicing neuroradiologists will ind helpful and informative. As with the irst edition, this book is image-rich with hundreds of color graphics and gross pathology examples that inform the thousands of new, up-to-date images. I’ve expanded on my trademark summary “blue boxes” that are scattered throughout the text, allowing for a quick review of the essential facts.
