Preface:

Plastic surgery is a vast specialty, encompassing the management of diseases and conditions from head to toe, in patients who are both young and old. With such a diverse and overarching surgical discipline, it comes as no surprise that our textbooks read like encyclopedias with seemingly limitless pages of detail and descriptions. An entire bookcase can easily be filled with textbook after textbook attempting to cover the topics that we encounter as plastic surgeons. As a student, resident, or young attending, the vastness of plastic surgery can become overwhelming, and it is easy to get lost in the details. One quick look at the six volumes required to cover our specialty in Plastic Surgery 3rd Edition alone can generate a tremendous amount of heartburn. This is particularly true during examination time, be it the in-service examination for trainees or the written examination for plastic surgery certification. I vividly remember the anxiety surrounding the exam preparation process and my disbelief at the lack of a “go-to” plastic surgery review book highlighting frequently tested, high-yield facts for the in-service exam and written boards. It seemed that all other specialties had one: a handy companion book with tattered pages of fruitful facts, figures, and illustrations that covered the broad topics and themes of the specialty, perfect for focused studying and examination review. The purpose of this book is to fill that void and serve as a trustworthy companion to students, residents, and young plastic surgeons alike. This book is not intended to be a replacement for your favorite plastic surgery textbooks or “encyclopedias,” nor is it meant to cover the minutiae of our specialty. Instead, it is intended to provide a foundation of plastic surgery fundamentals and themes upon which further details can be added. It is also intended to serve as an added resource for treating the anxiety and heartburn of examination preparation by providing a useful, relevant, and easily accessible collection of high-yield topics and facts covering plastic surgery. The book consists of 30 chapters, conveniently designed to align with the defined topics that are frequently covered in plastic surgery examinations. Within each chapter are numerous pearls of wisdom and abundant facts in bulleted format that cover some of the most frequently tested topics.

Moyamoya vasculopathy (MMV) is a rare cerebrovascular disease that is characterized by bilateral progressive steno-occlusion of basal cerebral arteries, with the emergence of coexisting abnormal net-like vessels. In moyamoya disease, MMV is the single manifestation, whereas in moyamoya syndrome or quasi-moyamoya, MMV is associated with a potentially underlying disease such as a genetic disorder or other coexisting pathology. Although MMV is most frequent in Asian countries, it is ranked among the most frequent causes of stroke in children and adults across the world. The incidence of MMV is on the rise due to increasing awareness of the disease. The relevance of surgical treatment of moyamoya disease by way of bypass revascularization is undisputed, which is in contrast to the surgical treatment of atherosclerotic carotid artery occlusion. The main aims of revascularization are to restore the blood supply to stabilize cerebrovascular hemodynamics and to regress the fragile moyamoya vessels in order to prevent bleeding. A successful improvement or normalization of cerebral hemodynamics will then result in secondary stroke prevention and improved neurological or neurocognitive outcome. Consequently, bypass surgery for MMV has become an integral part of the clinical practice of many microvascular neurosurgeons around the world. While the role of bypass surgery is well accepted, a versatile range of surgical techniques and strategies exists in the field, which makes it difficult to determine and appreciate the subtle nuances of the varied surgical strategies. Therefore, it seemed logical to create an instructive manual for neurosurgeons with a step-by-step guide to the surgical techniques. The focus of this book is on introducing neurosurgeons (and other physicians involved in the treatment of these patients) to the different surgical techniques, to the inherent strengths and weaknesses of each technique, and to the surgical considerations that need to be kept in mind. The management of surgical disorders requires not only the application of technical skills and training in the basic sciences to the problems of diagnosis and treatment but also a genuine sympathy and indeed love for the patient. At their first contact, the surgeon must gain the patient’s confidence and convey the assurance that help is available and will be provided. The surgeon must demonstrate concern for the patient as a person who needs help and not just as a “case” to be processed. This is not always easy to do, and there are no rules of conduct except to be gentle and considerate. Most patients are eager to like and trust their doctors and respond gratefully to a sympathetic and understanding person. Some surgeons are able to establish a confident relationship with the first few words of greeting; others can only do so by means of a stylized and carefully acquired bedside manner. It does not matter how it is done, so long as an atmosphere of sympathy, personal interest, and understanding is created. Even under emergency circumstances, this subtle message of sympathetic concern must be conveyed. Eventually, all histories must be formally structured, but much can be learned by letting the patient ramble a little. Discrepancies and omissions in the history are often due as much to overstructuring and leading questions as to the unreliability of the patient. The enthusiastic novice asks leading questions; the cooperative patient gives the answer that seems to be wanted; and the interview concludes on a note of mutual satisfaction with the wrong answer thus developed.

The Carilion Neurosurgery Department has afforded me the opportunity to focus on degenerative spine surgery, which is, and has been for some time, the area of my primary interest. The learning curve in the care of the painful degenerative lumbar spine is a long one. This is partly because the pathology in any given patient has a singular character. The anatomy of that pathology always varies in some degree, so that it will not be precisely alike any other. Furthermore, the multi-variant clinical factors in any patient can never be the exact same as in any other patient. The experienced surgeon therefore should have significant instructive value, clinically and operatively, to the young surgeon willing to learn what is still, to a large degree, an art. This book is not primarily meant to be a technical treatise. For instance, it is not so concerned with the specifics of the technique in the various types of lumbar fusion. It is more concerned, however, with identifying the patient who needs a fusion, and why a particular fusion method for that individual patient is the best option. And yet as degenerative lumbar spine surgery mainly concerns itself with various techniques of neural decompression, the principles of these methods will be presented, some of which may not be generally understood and practiced.
