  1. I’m finding this book (Deja Review USMLE Step 2 CK) very useful while studying for Step 2. I’ve gone through two sections so far (OB/GYN and Peds) and have found that it covers material that isn’t covered in USMLE World questions. It was recommended to me by a classmate who has taken Step 2 and then worked as an instructor in the Pass Program.
    The only bad thing about it is that it requires “unaided recall.” That is, you cover up the answers on the right side of the page and have to come up with the answer to the question on the left side of the page on your own. That’s more difficult than recognizing (by “aided recall”) the correct answer among five possible answers in a multiple choice test format like USMLE World. It’s also more time-consuming. However, you end up knowing the material better since you can produce it without aid.
  2.  I tried and tried and tried to read this book, but my eyes just glossed over the pages. It was more like lists of unexplained facts than recall. It’s more due to the way I study – I’m not a crammer. Look at a few example pages to see if it’s for you. You will be surprised at how boring this book is to read. I used Step 2 Secrets and liked it a lot more. I also just got my results back from Step 2 and am very pleased.

Got this book to study for Step 2 CK with about 3 weeks before the exam.
Pros:
-Easy to read
-High-yield information in boxes on the side
-Logical groupings of diagnoses together
-Common pitfalls of students outlined
Cons:
-Seems geared towards the average test-taker. If you are gunning for a high score, this book does NOT include a lot of the more esoteric material. Specifically, multiple questions in the newest NBME cannot be answered by looking at this book
-Inconsistent level of detail for certain diagnoses. Too much for some, too little for others
-Paucity of mnemonics or memory aids

Learning about ECG interpretation from books such as The ECG Made Easy or The ECG in Practice is fine as far as it goes, but it never goes far enough. As with most of medicine there is no substitute for experience, and to make the best use of the ECG there is no substitute for reviewing large numbers of them. ECGs need to be interpreted in the context of the patient from whom they were recorded. You need to learn to appreciate the variations of normality and of the patterns associated with different diseases, and to think about how the ECG can help patient management. Although no book can be a substitute for practical experience, 150 ECG Problems goes a stage nearer the clinical world than books that simply aim to teach ECG interpretation. It presents 150 clinical problems in the shape of simple case histories, together with the relevant ECG. It then invites the reader to interpret the ECG in the light of the clinical evidence provided, and to decide on a course of action before looking at the answer. Having seen the answers, the reader may feel the need for more information, so each one is cross-referenced to The ECG Made Easy and/or The ECG in Practice.
The ECGs in 150 ECG Problems range from the simple to the complex. About one-third of the problems are of a standard that a medical student should be able to cope with, and should be answered correctly by anyone who has read The ECG Made Easy. A junior doctor, specialist nurse or paramedic should get another third right, if they have read The ECG in Practice. The remainder should challenge the MRCP candidate. As a very rough guide to the level of difficulty of each problem, each answer is graded using stars (see the summary box of each answer): one star represents the easiest records, and three stars the most difficult. The ECGs are arranged in random order, not in order of difficulty, to maintain the reader’s interest. Readers are invited to attempt their own interpretation before looking at the star rating – after all, in a real-life situation one never knows which patient will be easy and which will be difficult to diagnose or treat. It is some 8 years since the first edition was published. Since then, I have been grateful to receive helpful comments and suggestions as to how to improve the publication. It is maintained that good cardiac pathology knowledge is always to be balanced alongside awareness of embryological and developmental matters, current therapeutics, modern imaging techniques, and a reasonable understanding of cardiac electrophysiology. These subjects have kept their place in the publication, along with the other standard cardiac pathology chapters. The rising use of postmortem radiology prompts its appearance within the autopsy chapter. As always, thanks go to the chapter authors for their unfaltering dedication to the project. They have built upon the previous edition and the work of other practitioners. Likewise, I am indebted to my colleagues who have shared their interesting cases, thereby adding to the photomicrographs and macroscopic images in the publication. The heart is a complex, folded and hollow muscular structure situated just to the left side of the mid-low sternum when viewed from the front of the body, being enclosed by the pericardial sac and joined to the great vessels [1–3]. It develops its shape from embryological folding of the cardiac tissues (see chapter “Embryology of the Heart”) [4]. The surface/external landmarks of the cardiac tissues, viewed from the front of the body, are the right and left parasternal second intercostal spaces down to the right sixth costal cartilage with the apex of the heart being in the fifth left intercostal space mid-clavicular line. To appreciate any cardiac disorder one must first appreciate normality. This simple statement is deceptive since the heart is an organ with a complex 3-dimensional architecture, along with mechanical functionality. It is made mostly of muscle, although the muscular tissue varies in format across the chambers, with a dynamic microscopic layout and an elegant electrophysiological function. There are non-muscular tissues are also present within the heart, namely blood vessels, nerves, connective tissues and fat. These are normal, but excessive amounts of fat and fibrous tissues may point to a pathological process, as discussed later in this book.

Congenital heart disease (con, together; genitus, born) is often viewed as a group of gross structural abnormalities that are present at birth. Although not incorrect, this definition requires elaboration and qualification. Prenatal diagnoses have been possible for more than 25 years.2 At about embryonic day 20 in the human fetus, progenitor cells within the mesoderm become committed to a cardiogenic destination.3 Most malformations compatible with 6 months of intrauterine life permit live offspring at term. A given malformation may exist in relative harmony with the fetal circulation, only to be modified considerably, at least physiologically, by dramatic circulatory changes at birth.4 Weeks, months, or years may then elapse before an anomaly reveals itself as characteristic or typical. A functionally normal aortic valve that is congenitally bicuspid at birth may take decades to fibrose, calcify, and present as overt aortic stenosis. Conversely, a malformation may disappear, as is the case with spontaneous closure of a ventricular septal defect. Congenital malformations of the heart and circulation are not fixed anatomic defects that appear at birth but instead are anomalies in flux that originate in the early embryo, evolve during gestation, survive the dramatic circulatory alterations at birth, and change considerably during the course of extrauterine life.
Congenital heart disease was once the exclusive and legitimate domain of pediatrics, but survival patterns have changed appreciably. 5,6 In the United States today, there are more adults with congenital heart disease than there are infants and children; these new generations of patients are best cared for by specially trained cardiologists.7 Clinical presentations, especially in adults,8,9 can be exceptionally complex.10–12 Furthermore, certain defects that are actually or potentially of functional significance are not gross structurally, such as congenital complete heart block, either isolated (see Chapter 4) or with congenitally corrected transposition of the great arteries (see Chapter 6) or left isomerism (see Chapter 3); Wolff-Parkinson-White preexcitation, either isolated or complicated (see Chapter 13); absence of a sinus node, as with a superior vena caval sinus venosus atrial septal defect (see Chapter 15) or left isomerism (see Chapter 3); ventricular tachycardia with the long QT syndromes13; or arrhythmogenic right ventricular dysplasia. Still other abnormalities, such as Marfan syndrome, which is the result of mutations in the gene that encodes fibrillin-1, are actually or potentially of functional significance but are not necessarily manifest at birth as gross structural malformations and, as a matter of convention, are usually not classified as congenital.14 And a handful of odd defects tend to escape inclusion, such as congenital kinking of the internal carotid artery.

The last two decades we have witnessed significant progress in the understanding of atrial fibrillation, from its pathophysiology to its complex and diverse etiologies and relation to most devastating complication, stroke. Despite several recent breakthroughs on novel oral anticoagulation, antiarrhythmic agents and interventional procedures, we are still losing the battle or the management remains at least unsatisfactory. This in part may be due to the evolving nature of this rhythm disorder and the fact all atrial fibrillations are not the same, thus we are not dealing with the same entity. Despite many guidelines and consensus from many medical societies as well as several large texts on the subject we feel that there exists a gap between academic centers and practitioners who indeed see 80% of patients with AF. This short text is therefore designed to provide the most recent evidence-based information and how to deal with patients with Atrial Fibrillation. The target audiences of this book are primary physicians, Emergency room doctors, hospitalists, internists, and generally cardiologists who care after the majority of these patients. We would like to thank our colleagues who provided their up to date evidence-based manuscripts. Atrial fibrillation is an arrhythmia that is easy to recognize but difficult to treat. Not having the right focus may be deleterious for patients suffering from this arrhythmia.

Over the past decades an increasing number of studies have been performed and several guidelines have been issued to improve management of atrial fibrillation. Initially, the main focus of management was on controlling the rhythm by cardioversion and antiarrhythmic drugs, shifting to antithrombotic treatment in the early nineties of the previous century. However, patients with atrial fibrillation are not only threatened by stroke but even more by heart failure and cardiovascular death. Therefore, the large rhythm versus rate control studies—including for instance AFFIRM, AF-CHF and ATHENA—appropriately used major adverse cardiovascular and cerebrovascular events as an endpoint. As a consequence, we nowadays know very well that upstream therapy—which is nothing more and nothing less than adequately and aggressively treating the underlying cardiovascular diseases associated with atrial fibrillation— is of utmost importance to prevent further harm. Eventually, all this helped to reduce morbidity and mortality rates in patients with atrial fibrillation. Notwithstanding the above, daily management of atrial fibrillation remains a challenge. In addition, early identification of high risk patients—even going beyond the well-known clinical prediction schemes of CHA2DS2-VASc and HAS-BLED—and early personalized treatment to prevent atrial fibrillation progression are new dots on the management horizon.

Prof. Vida is a surgeon in the Department of Cardiac, Thoracic and Vascular Sciences at the Pediatric and Congenital Cardiac Surgery Unit at the University of Padua Medical School in Italy. He spent 2 years with Prof. Aldo Castaneda in Guatemala and almost 2 years with Prof. Del Nido at Children’s Hospital Boston, Harvard Medical School. Prof. Vida published more that 150 peer-reviewed paper and edited several books and book chapters. He has also been invited by numerous societies to present the results of his experience. His major research areas include neonatal congenital heart surgery, minimally invasive surgery, congenital heart disease in adults.
